Who Is Eligible:

- St. Gerald Catholic, 5 day, and Religious Education (CCD Students)
- 1st grade through 8th grade

How to Register:

- Complete this registration for (one for each player)
- Payment via Paypal is preferred. If you would prefer to use a check please drop it off at the school office.

Registration Fee:
$40.00 per player |

Uniform Fee:
$35 per player (If you need a uniform this will be paid seperately during the uniform sizing) |

Uniform Note:
Uniform fee will only be charge when you need a new uniform. Uniforms will be utilized for multiple seasons. |

DEADLINE:
All registration forms must be complete and received no later than Tuesday, May 22, 2012. Payment must
be received by Tuesday, May 22, 2012. Feel free to email soccerregistrar@stgerald.org to confirm receipt of registration form and fee.

League Information: St. Gerald's Soccer Club is participating member of the Catholic Youth Soccer
League (www.CYSL.net) and is regulated by the Nebraska State Soccer Association
(www.nebraskasoccer.org). As a member of the CYSL, we are held to be compliant with all NSSA
and CYSL policies and procedures.

A key regulation is team composition. Teams are divided into age groups and based on date of birth. Due to this regulation, a student athlete
cannot play in a lower age bracket. They may be able to move up to the next age bracket if his or her abilities allow.

St. Gerald Soccer Club Release/Waiver
I/We, the parents of the above named child, a participant in the St. Gerald soccer program, hereby give my/our approval
of our child's participation in all St. Gerald Soccer Club activities and events. By submitting this registration and providing
payment to enter the league I/We assume all risks and hazards incidental in such participation, and I/We indemnify and agree to
hold harmless, St. Gerald Soccer Club, Catholic Youth Soccer League, its organizers, sponsors, all qualified staff, all persons
and all claims for negligent acts and omissions. Coaches, managers, referees, their assistants, or anyone who prepares any playing
field shall not be liable for any injury or damages to any participant in the St. Gerald Soccer Club, which results from the negligence
of any of the above listed individuals. I certify that I carry adequate accident and liability insurance presently in force covering
my child and I will continue such coverage at my expense while he/she participates in the club's program.
